Mobile Outreach Worker

Position Description:

The Mobile Outreach Worker will provide community-based services for women experiencing homelessness with a history of substance use. Services will be provided throughout the Central San Diego region. Utilizing an integrated care approach, the Mobile Outreach Worker will provide outreach/engagement services and brief case management interventions to quickly assess women’s needs and link them with Rachel’s Women’s Center or other recovery oriented, community based resources, where they will be offered assistance based on their individual needs. The Mobile Outreach Worker also provides these engagement and case management services for women at Rachel’s Women’s Center when needed.

Essential Functions:

 MOBILE OUTREACH & ENGAGEMENT

    • Conducts mobile/street outreach, building rapport and developing trust with women experiencing homelessness
    • Provides information about available resources and assists women in identifying the services that are right for them at this point in time
    • Assists women in identifying and alleviating barriers to services including housing, behavioral and/or physical health care or other community services based on her individual needs
    • Assists women with establishing goals within 30 days of enrollment
    • Supports women in meeting their goals and updates goals at least every 90 days (Case Management)
    • Completes basic intakes and PROMIS forms with women served
    • Engages in Diversion conversations and, when appropriate, conducts a VI-SPDAT (Vulnerability Index-Service Prioritization Decisions Assistance Tool) to connect women with housing resources  
    • Provides transportation to Rachel’s Women’s Center or other services
    • Assists women with obtaining birth certificates, social security cards or other documents necessary to access benefits and services
    • Reconnects participants with their established service providers when those relationships exist
    • Advocates with outside agencies and Catholic Charities programs to ensure women’s needs are being appropriately met

RELATED RESPONSIBILITIES

  • Remains mindful of safety at all times
  • Builds and maintains collaborative working relationships with colleagues in other social service agencies including detox and substance use treatment programs, walk in mental health clinics and other co-occurring disorders programs, primary care clinics and outreach programs including Downtown Partnership, HOT, PERT, NAMI. Possesses a strong working knowledge of eligibility criteria for these programs and recommends the best options based on each woman’s individual needs.
  • Ensures that mobile vehicle is always ready for use (clean and has a full gas tank)
  • Promptly reports mobile vehicle maintenance needs immediate supervisor so it can be serviced according to manufacturer’s standards
  • Completes mileage logs before the end of each workday
  • Performs other duties as requested

     INFORMATION MANAGEMENT

    • Accurately and completely records all services provided using the program’s internal data collection system, ClubHOMS (when available) and Clarity HMIS
    • Prepares and submits all required reports and assigned paperwork in a timely and accurate manner 

     

    PROGRAM SPECIFIC EXPECTATIONS:

    • Develop and maintain comprehensive, up to date knowledge of community resources that address the needs of people experiencing homelessness.
    • Establish and nurture collaborative relationships with colleagues from other agencies providing similar or complimentary services
    • Applies a trauma informed, safety focused approach to service delivery and supports participants’ efforts to obtain self-reliance
    • Work collaboratively with coworkers from Catholic Charities Homeless Programs and throughout the Agency
    • Attend Rachel’s staff meetings and participate in training opportunities as requested

     

    QUALIFICATIONS:

    • BSW/BA/BS Degree or equivalent life/work experience
    • Preferred: MSW or Master’s in Behavioral Health related field or equivalent life/work experience
    • Minimum two years’ experience overseeing services for people experiencing homelessness, and/or adults living with co-occurring serious mental illness or substance abuse. Experience with women’s issues and services
    • BSW/BS/BA degree in a human services, social, behavioral sciences or related field or Associate degree and minimum two years case management/navigation or outreach experience
    • Preferred: MSW or Master’s in Behavioral Health related field or equivalent life/work experience.
    • At least one years’ experience serving people experiencing homelessness and/or co-occurring disorders. Experience in women’s issues and services
    • Experience applying trauma informed approaches to service delivery, motivational interviewing, stages of change model and harm reduction. Solid working understanding of and demonstrated ability to apply trauma informed principles to complex, unpredictable situations.
    • Understands and embraces a Housing First approach to resolving homelessness
    • Fluency in English required, fluency in Spanish or other threshold language preferred
    • Strong computer skills and ability to become proficient with data base software
    • Ability to read an interpret documents such as policies, safety rules, operating instructions, and procedure manuals
    • Ability to multi-task, organize, and prioritize
    • Ability to multi-task and remain patient with participants and co-workers
    • Ability to work independently and effectively as part of a team
    • Candidates with lived recovery experience and/or those who experienced homelessness are encouraged to apply
    • Computer proficient in MS Office and the ability to learn and accurately utilize electronic documentation systems

      PHYSICAL REQUIREMENTS:

      1. Ability to walk for up to 8 hours per day with intermittent standing, bending, squatting, or climbing
      2. May be asked to lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds
      3. Specific vision abilities required by this job include close vision

      Work Schedule: Full-time (40 hours/ week), M-F 8:00 AM – 5:00 PM

      Salary: $20.00 Per Hour
